If your name or the name of a business you own is included in the list on the prior page, please fill out the updated contact information below so we can update our records. We will reach out by email or phone within two business days to verify that the account information has been updated and we will re-issue any equity checks we have on file to your new address. Any future equity redemptions will be mailed to this new address as well.
